A variety of forms are available for viewing on-line, or for saving on your system for off-line viewing and printing. The HTM form is interactive and may be printed out for your records before being submitted.

 

These forms may be submitted in various ways, after downloading:

1. Print the form, fill in by hand or typewriter, mail to address on the form. (This is the only choice with PDF files.)
2. A word processor can be used to fill in the RTF or DOC type form, save as RTF or DOC file, send as e-mail attachment to your District Office.
3. MS Excel can be used to fill in the fields of the XLS type form, save as XLS file, send as e-mail attachment to your District Office and perhaps to the Conference Treasurer.
4. If the form is in HTM, then make the entrees and click the "Submit" button.


Some forms are available as RTF files:

Most word processor programs are capable of handling .rtf files. (hint: To avoid the problem of the underlined areas extending, because you are trying to edit/insert text, you need to be in "overwrite mode [OVR]").

In MS Word 2000, you can enter OVR under tools/options/edit/overwrite. Once in OVR, you will notice OVR highlighted in the "status bar" at the bottom of the page. You can toggle in and out of OVR by double-clicking on the highlighted OVR.

It is suggested that after the RTF file is downloaded, it should be saved on your system, and then later opened in MS Word or whatever Word Processor you are using. The Word Processor that is executed automatically by your browser at the time of download may have less functionality than what you may be accustomed to.

If your system does not have a "Viewer", capable of viewing (not editing) RTF files, there is one available, for free, from the following Web Page. You can print hardcopy with this "Viewer".

If you don't have a word processor program, there are free ones available which can read and write RTF (and DOC) formats. One of the most acclaimed is AbiWord. Another option is the office suite, OpenOffice.org.

This file type is in HTM Form style allows an interactive input, this results in a structured e-mail that is submitted/sent to the office of the District Superintendent.
